GreatCare OBGYN requires written parental consent for the treatment of any minor. If your child is under the age of 18 and will be seen without you present, you will need to sign a Parental Consent form and it will need to be presented at each visit before your child will be seen by Dr. Andrews. Parental Consent forms are located in our Forms section on our website and all information completed on behalf of the minor patient will be verified prior to services rendered.
Although, we are extremely diligent in the verification of information submitted, we must place a caveat of awareness that GreatCare OBGYN is not responsible for the falsification of any information submitted on consent forms presented to our office.
