Release of Medical Information
In accordance with state and federal laws, GreatCare OBGYN will not release any medical or personal information to anyone, including but not limited to your spouse, without your specific written consent. This is applicable to any patient over the age of 18 years old. For release of information on patients under the age of 18, the parent or legal guardian will be required to sign a release form. Please keep in mind, our policies regarding these matters are governed by law, and are therefore not flexible; no exceptions will be made.
Patient medical records are the property of GreatCare OBGYN. If you would like a copy of your medical records to provide to another physician, you will be required to sign a Medical Records Release Form. This form is located in our Forms section of our website. We release records directly to you for you to provide to your physician as you choose. Your records can either be picked up at our office or we will mail them to you Certified, Return Receipt Mail. Please allow up to 15 business days for the processing of your records.
Per the Texas Medical Board Rules; Section 165.2(e), a Physician or Practice responding to a request for such information shall be entitled to receive a reasonable, cost-based fee for providing the requested copies of medical records. This fee varies based on format and amount of documentation provided. GreatCare OBGYN does charge a fee for copying your medical records, and this fee is required to be paid before we release any information to you. No exceptions will be made.
